GENERAL TERMS AND CONDITIONS

for the provision of health care in a health facility

1. Introductory provisions

These general terms and conditions (hereinafter referred to as "VOP") regulate the rights and obligations between ATRIM SK s. r. o., Smaragdová ul. 1, 851 10 Bratislava, ID No.: 55482261 (hereinafter referred to as the "Provider") and natural persons - clients (hereinafter referred to as the "Client") to whom the Provider provides health care and related services.

The provider operates a private medical facility that provides health care in the following disciplines:

- dermatovenerology (for adults and children),

- plastic surgery,

- aesthetic dermatology and

- Cosmetology.

Health care is provided on an outpatient basis and exclusively for direct patient reimbursement. The clinic does not have contracts with health insurance companies.

2. Definitions

Health care - procedures, examinations and medical treatment performed by the Provider's authorised medical staff.

Superior services - services that go beyond routine health care, including aesthetic procedures and services related to client comfort.

Client - a natural person who uses the Provider's services on the basis of a verbal, written or electronic arrangement.

Contractual relationship - is established by the Client's appointment for an examination (by phone, e-mail, online or in person) and acceptance of these VOP.

Remuneration - the price for the services provided according to the current price list published in the clinic premises or on the website.

3. Provision of health care

Health care is provided professionally, ethically and with dignity, according to current medical knowledge, only on the basis of the informed consent of the client.

Before any procedure or examination, the Client will be informed about its purpose, course, possible risks and alternatives. The Client has the right to freely decide whether to consent.

The client is obliged to truthfully inform the medical staff about his/her health condition and to provide the necessary information that may affect the treatment.

4. Ordering and cancellation conditions

You can make an appointment for an examination in the following ways:

- by phone: +421 917 080 080,

- E-mail: dermarevolta@dermarevolta.com,

- in person at the clinic,

- via the online form at www.dermarevolta.com,

- or through Dermarevolta's social media platforms Facebook and Instagram (via messages or comments).

In case the Client cannot attend the agreed appointment, he/she is obliged to notify the Provider well in advance.

Cancellation conditions are governed by the Provider's current price list and are as follows:

Reason for cancellation Amount of cancellation fee

Cancellation more than 24 hours before the date 0 €

Cancellation less than 24 hours before the deadline 50 % of the price of the scheduled performance

Failure to arrive on time without cancelling 100 % of the scheduled performance price

The Client is obliged to pay the cancellation fee at the request of the Provider - in cash, by card or by transfer to the account. The Provider has the right to send a written request for payment in case of default.

5. Payment terms

The client pays for all services provided in full, according to the current price list. Payment can be made in cash, by credit card or by bank transfer on the basis of an invoice.

The price list is available in the clinic and on the Provider's website. The Provider reserves the right to change the price list. The Provider shall inform about the change in an appropriate manner.

6. Processing of medical documentation

The provider shall keep medical records of the health care provided in accordance with applicable law. The client has the right to inspect the documentation, to request an extract or a copy and to object to incorrect data.

Medical records shall only be provided to authorised persons and bodies as required by law.

7. Lodging complaints

The client has the right to complain if he/she believes that the services have not been provided correctly or professionally.

A complaint can be lodged:

- in writing at the clinic's registered office or

- by e-mail to: dermarevolta@dermarevolta.com

Complaints shall be dealt with within 5 working days. The responsible person is Anna Melnychuk, Director.

8. Privacy Policy

The Provider processes the Clients' personal data in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council (GDPR) and Act No. 18/2018 Coll. on the Protection of Personal Data.

9. Liability and Dispute Resolution

The Provider is not liable for damages caused by false or incomplete information provided by the Client.

Any disputes between the Client and the Provider shall be resolved preferably by agreement. If no agreement is reached, the competent court in the Slovak Republic shall decide.

10. Final provisions

These General Terms and Conditions come into force on the date of their publication on the Provider's website. The Provider is entitled to change them unilaterally.

The Client shall be deemed to be aware of and agree to these terms and conditions at the moment of making an appointment for an examination or otherwise using the Provider's services.

These terms and conditions shall survive the termination of the contractual relationship with regard to the settlement of any liabilities.
